
ABOUT HOP
The Healthy Opportunities Pilot (HOP) is an innovative new approach to test and evaluate non-medical interventions related to housing, food, transportation and interpersonal safety and toxic stress to eligible Medicaid enrollees. This Medicaid pilot program was developed by North Carolina Department of Health and Human Services (NCDHHS) with a goal to improve health and reduce health care costs by investing in evidence-based interventions.
Community Care of the Lower Cape Fear was selected as one of three Network Leads in North Carolina to pilot the Healthy Opportunities Program. There are three primary goals of the pilot:
- Enrollees can access services in a timely manner that meets their needs and improves their health
- Diverse and equitable participation through human services organizations for all enrollee
- High-quality enrollee centered services through community organizations
Pilot Services
Healthy Opportunities Pilot will cover the cost of 29 defined interventions based on their potential to improve health outcomes and lower health care costs by addressing non-medical needs to address housing, food, transportation, interpersonal violence and/or toxic stress needs of qualifying Medicaid enrollees. The Healthy Opportunities Pilot provides 29 approved non-medical interventions based on their potential to improve health outcomes and lower health care costs. HOP is the nation’s first comprehensive program to test and evaluate the impact of providing non-medical interventions related to food, housing, transportation, and interpersonal safety to high-needs beneficiaries.
The Cape Fear HOP Network
Area nonprofits and community partners applied to participate in the HOP pilot and become part of the Human Service Organization (HSO) Network. This HSO network will deliver services to high-risk Medicaid beneficiaries approved to participate in the pilot in six counties (New Hanover, Pender, Brunswick, Columbus, Onslow, and Bladen).
