On May 15th the Healthy Opportunities Pilot expanded to eligible enrollees within Tailored Care Management (TCM). Through TCM, Behavioral Health I/DD Tailored Plan beneficiaries have a single designated care manager supported by a care team to provide care management that addresses all of their needs including physical health, behavioral health, I/DD, traumatic brain injuries (TBI), pharmacy, long-term services and supports (LTSS) and unmet health-related resource needs. In the Cape Fear HOP region, pilot services are available to eligible Trillium Health Resources members.
